8/4/2023 EnchantaVaughn Approved Behavior: As the session began, Anthony took a few minutes to gather himself before going to talk to QMHP-C. QMHP-C engaged mother in a discussion to determine Anthonyâs behaviors since the last session. Mother verbalized to QMHP-C she continues to struggle with Anthony managing his anger and not listening to her when she asks him to comply with simple request. Behavior: Anthony displayed age-appropriate skills and impulsive, hyperactive behaviors were not present at this time and need redirection from QMHP-C in an effort for him to get back on track with the session. Behavior: Anthony was observed during the session with an upset look on his face and choosing to distance himself away from his counselor when inquired about why he didnât want to talk or share anything today. Intervention: QMHP-C will meet with Anthony daily to discuss progress and expectations that are working and not working in the home weekly. QMHP-C inquired of Anthonysâ current mood. QMHP-C utilized active listening skills as Anthony spoke of his current mood. QMHP-C utilized active listening as information was conveyed. QMHP-C will provide Anthony with support and encouragement. QMHP-C will continue to build rapport. QMHP-C communicated with Anthony on displaying appropriate control. Intervention: QMHP-C reflected with Anthony on his day, reviewing both good and bad experiences. QMHP-C provided a summary of the session, to reassure Anthony had clear knowledge of the purpose of session. QMHP-C directed Anthony to verbally express positive affirmations. QMHP-C will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activity. QMHP-C will educate Anthony on behavior modification to help Anthony identify what behavior he needs to decrease and avoid daily. Intervention: QMHP-C discussed actions and consequences. QMHP-C will teach Anthony necessary coping skills to decrease impulses significantly. QMHP-C encouraged Anthony to take small breaks and then process forward. QMHP-C encouraged Anthony to be mindful of others and utilize role reversal techniques to put himself in their shoes. QMHP-C monitored Anthony until the end of session. Response: Anthony was able to reevaluate himself and display a more positive attitude toward peers and authority. Anthony was observed having positive interactions with everyone in the home. Anthony demonstrated responsibility and leadership by helping his mom around the house. Anthony implemented his plan for restoring relationships, and this was reviewed. Anthony demonstrated expectations and responded well to feedback. Anthony continued to interact in a positive manner with his mother and counselor. Response: Anthony took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time. Anthony nodded in agreement. Anthony was observed displaying age appropriate behaviors and was productive and studious throughout the day. Anthony demonstrated clear understanding of objectives. Anthony expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Response: Anthony completed his activity without any interruptions. Anthony was acceptive of prompts and encouragement given by QMHP-C. Anthony was praised for his cooperation and modeling appropriate behavior. Anthony nodded in agreement and promised to do what was asked by his mother and mental health professional. Anthony appeared to be learning from his mistakes and correcting them.
